During an NRC inspection conducted on April 1-30,1988, a violation of NRC l requirements ras identified. The violation involved operation without an i approved procedure. In accordance with the "General Statement of Polic.y and ,

Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1988), the I violation is listed Lelow- I l

Deficient Procedure l l

Licensee Technical Specification AC 7.4.a.1.equires, in part, that I applicable procedures be established and implemented for activities recomended in Appendix A of Regulatory Guide 1.33, November 1972.

Appendix A of Regulatory Guide 1.33, November 1972, requires procedures for startup, operation, and shutdown of safety-related systems.

Contrary to the above, neither System Operating Procedure 23, Issue 31, ,

"Helium Purification System," nor System Operating Procedure 25, Issue 13, "Nitrogen System," address the valve manipulations required to fill the liquid nitrogen storage tank. This leck of procedural guidance contributed to miso;seration of nitrogen storage valves that, inturn, re.sulted in an unplanned release of radioactivity on April 26, 1988.

This is a Severity Level IV violation. (Supplement I.D.)(267/8810-01) i Pursuant to the provisions of 10 CFR 2.201, Public Service Company of Colorado l is hereby required to-submit to this office within 30 days of tht. date of the letter transmitting this Notice, a written statement or explanation in reply, including for each violation: (1) the reason for the violation if admitted, (2) the corrective steps which have been taken and the results achieved, (3) the corrective steps which will be taken to avoid further violations, and (4) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.
