APPENDIX A.
NOTICE OF VIOLATION Public Service Company of Colorado Docket:
50-267 Fort St. Vrain Nuclear Generating Station License: DPR-34 During an NRC inspection conducted on June 23-27, 1986, a violation of '
NRC requirements was identified. The violation involved the failure to take prompt corrective action which was effective for conditions which were identified as adverse to quality.
In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1986), the violation is listed below:
Prompt and Effective Corrective Action Criterion XVI of Appendix 8 of 10 CFR Part 50 and the licensee's approved quality assurance program require that conditions adverse to quality be promptly identified and corrected.
Contrary to the above, certain conditions, adverse to' quality, which had been identified, had not been corrected.
Examples are:
Nonconformance reports 37, 38, and 40, issued as a result of the 1983 biennial review of quality assurance, were not documented as being resolved.
Corrective action report (CAR)81-204, dated June 28, 1981, remained unresolved and unclosed.
CAR 83-082 was closed without the response addressing the deficiency stated therein.
This is a Severity Level IV violation.

Pursuant to the provisions of 10 CFR 2.201, Public Service Company of Colorado is hereby required to submit to this office within 30 days of the date of the letter transmitting this Notice, a written statement or explanation in reply, including for each violation:
(1) the reason for the violation if admitted, the corrective steps which have been taken and the results achieved, the corrective steps which will be taken to avoid further violations, and the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.
