NOTICE OF VIOLATION Omaha Public Power District Docket: 50-285 Fort Calhoun Station License: DPR-40 During an NRC inspection conducted on June 1 through July 15, 1987, a violation of NRC requirements was identified. The violation involved the failure to provide appropriate instructions for the modification of a safety-related system.

In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1987), the violation is listed below:

Criterion V of Appendix B to 10 CFR Part 50 requires that activities affecting quality shall be prescribed by documented in:;tructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomp;ished in accordance with these instructions, procedures, or drawings. 3 Section 5.1, " Control of Plant Design and Modifications," of the l'censee's Quality Assurance Manual and Procedure 50-G-21, " Station Modification Control," have been established to implement requirements of Appendix B and '

require that modification of equipment be performed in accordance with written procedures. ,

I Contrary to the above, during the 1987 refueling outage, the licensee {

performed a modification to the containment sump discharge line by 1 installation of a pressure gage and tubing without the use of a written and approved procedure.

This is a Severity Level IV violation. (Supplements)(285/8715-02)

Pursuant to the provisions of 10 CFR 2.201, Omaha Public Power District is hereby ,

required to submit a written statement or explanation to the U.S. Nuclear Regulatory Commission, ATTN: Document Control Desk, Washington, D.C. 20555 with a copy to the Regional Administrator, Region IV, and if applicable, a copy to the NRC Resident Inspector, within 30 days of the date of the 1atter transmitting this Notice. This reply, should be clea.ly marked as a " Reply to a. Notice of Violation" and should include for each violation: (1) the reason for the violation if admitted, (2) the corrective steps that have been taken and the results achieved, (3) the corrective steps that will be taken to avoid further violations, and (4) the date when full compliance will be achieved. If an adequate reply is not received within the time specified in this Notice, an order may be issued to show cause why the license should not be modified, suspended, or revoked or why such other action as may be proper should not be taken. Where good cause is shown, consideration will be given to extending the response time.
