O APPENDIX A NOTICE OF VIOLATION Omaha Public Power District Docket: 50-285 Fort Calhoun Station Operating License: DPR-40 During an NRC inspection conducted during the period March 13-31, 1989, and April 17-21, 1989, violations of NRC requirements were identified. These violations were the failure to perform safety evaluations of plant modifications, and the failure to take prompt corrective action of conditions adverse to quality.

In accordance with the " General Statement of Pelicy and Procedure for NRC Enforce--

ment Actions," 10 CFR Part 2, Appendix C (1988), the violations are listed below:

A. Violations Involving Changes In The Facility 10 CFR Part 50.59, requires that the licensee shall maintain records of changes in the facility. These records must include a written safety evaluation, which provides the bases for the determination that the change does not constitute an unreviewed safety question.

Contrary to the abeve, Omaha Public Power District failed to have a safety evaluation for each of the modifications listed below:

1. A spray shield was installed in the switchgear room over Load Center 1848.

The spray shield was attached to a safety-related cable tray on one end with the other end being supported by a conduit from the load center.

2. The diesel generator exciter brush housing inspection covers had been replaced with plexiglass covers.
3. Installations of a temperature inversion switch in Radiation Monitoring Panel AI-33B.
4. Unsupported wiring with no redundant channel separation in the RCS hot leg and cold leg temperature and steam generator pressure control room instruments.

This is a Severity Level IV violation. (Supplement I) (285/8901-05)

B. Failure to Take Prompt Corrective Action Criterion XVI of Appendix B to 10 CFR 50 requires that, " Measures shall be established to assure that conditions adverse to quality, such as failures, malfunctions, deficiencies, deviations, defective material and equipment, and nonconformances are promptly identified and corrected."

This requirement is implemented by Section 10.4 of the licensee's quality assurance plan, which states in Section 4.1.1 that, " Conditions adverse to quality, such as failures, malfunctions, deficiencies, deviations, defective material and equipment, and nonconformances, shall be promptly identified and corrected as soon as practicable."

Contrary to the above, the 'NRC . inspectors-found on April 19, 1989, an energized lighting circuit in the No.il diesel generator room which was routed outside of conduit with the energized wiring hanging on.the valves on the air' start receivers. This problem had been identified by the licensee.in'1987 and Maintenance Order 874799 was issued on October 14, 1987, to correct the problem. The M0 was subsequently lost or misplaced.

The' condition was again addressed in.hnuary 1989. Maintenance Order 890297 was istued on Janusry 10, 1989, to repair _the wiring, but the problem had not been corrected at the time of this inspection.
