NOTICE OF VIOLATION Omaha Public Power District Docket: 50-285 Fort Calhoun Station Operating License: DPR-40 During an NRC inspection conducted on April 2-6 and April 16-19, 1990, four violations of NRC requirements were identified. The violations involved inadequate assessment aids, detection aids, compensatory measures, and key control. In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1990) (Enforcement Policy), the violations are listed below:

License Condition 3.d of Amendment 42 to the Fort Calhoun Station Operating License, dated March 25, 1981, requires that the licensee maintain in effect ,

and fully implement all provisions of the Commission-approved physical security plan (PSP), including amendments and changes made pursuant to the authority of 10 CFR Part_50.54(p).

A. Key Control Section 5.2 of the PSP requires, in part, that all unused security keys be stored in a secure. location.

Contrary to the above, the inspector determined that the licensee failed to secure keys to two VAs when the keys were placed in an unsecured key depository within the control room.

This is a Severity Level IV violation. (Supplement III)(285/9025-01)

B. Assessment Aids Section 8.3.1 of the PSP requires, in part, surveillance of the protected area (PA) perimeter and isolation zones to be assessed by fixed and pan-tilt-zoom closed circuit television (CCli) cameras.

Contrary to the above, some cameras observed by the inspectors on April 3, 1990, provided inadequate assessment of parts of the PA perimeter and isolation zones due to insufficient camera resolution.

This is a Severity Level IV violation. (Supplement III)(285/9025-02)

C. Protected Area Detection Aids Section 8.1.1 of the PSP requires, in part, each microwave zone to be designed to detect an individual crawling, walking, running, jumping, or rolling within the zone of coverage and sufficient overlap to be provided between adjacent zones to eliminate detection deadspots.

Contrary to the above, on April 4,1990, inspectors observed that some microwave zones were not designed with sufficient coverage'and overlap to detect jumping over and crawling within those detection zones. -

This is-a Severity Level IV violation. (Supplement III)(285/9025-03)

D. Compensatory Measure Section 4.3.3 of the PSP requires, in part, that compensatory posts will r be adequate to ensure no degradation of security effectiveness occurs as a result of partial or complete system failure.

Contrary to the above, on April 4,1990, the licensee. failed to establish adequate compensatory posts required for a complete computer system failure; therefore, a degradation of security effectiveness existed during the computer outage.

This is a Severity Level IV violation. (Supplement 111)(285/9025-04)

Pursuant to the provisions of 10 CFR 2.201, Omaha Public Power District is hereby required to submit to this office, within 30 days of the date of the  !

= letter transmitting this Notice, a written statement or explanation in reply, including for each violation: (1) the reason for the violation if admitted, (2) the corrective steps which have been taken and the results achieved, (3) the corrective steps which will be taken to avoid further violations, and (4) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.
