NOTICE OF VIOLATION ,

! During an NRC inspection conducted on March 23 through May 3,1997, two violations of -

NRC requirements were identified. In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," NUREG-1600, the violations are listed below:  ;

A. Appendix B of 10 CFR Part 50 Criterion V, states, in part, that activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of -

a type apprcpriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings.

Standing Order S0-R-2, " Corrective Action Program," Revision 3, Step 2.2, states, I in part, that defective rnaterial and equipment, abnormal occurrences, and nonconformances affecting structures, systems or components require the initiation of a condition report. j Contrary to the above, the inspector identified that, on March 26,1997, l engineering personnel did not initiate a condition report when a body-to-bonnet i steam leak was identified on the steam trap inlet valve for the turbine-driven '

auxiliary feedwater pump steam supply line. ,

This is a Severity Level IV violation (Supplement 1)(285/97007-01). *

8. Technical Specification 5.8.1 states, in part, that written procedures and administrative policies shall be established, implemented, and maintained that meet or exceed the minimum requirements of Regulatory Guide 1.33.

Regulatory Guide 1.33, Appendix A, Section 9.a, states, in part, that maintenance '

that can affect the performance of safety-related equipment shall be performed in )

accordance with written procedures or documented instructions appropriate to the I circumstances. i Standing Order SO-M-100, " Conduct of Maintenance," Step 4.6.2, directs maintenance personnel to perform maintenance in accordance with approved documents.

Preventive Maintenance Order 9701947 directed maintenance personnel to blow down the raw water supply header flow transmitter sensing lines by opening the l drain valve to clear the lines. Step 6 of the preventive maintenance document L stated that, if the sensing lines could hot be cleared, initiate a maintenance work

document to document the deficiency.

l l l l Contrary to the above, in addition to opening the drain valve to clear the sensing lines, the inspectors found that on April 7,1997, maintenance personnel used nitrogen to blow down the sensing lines without initiating a maintenance work document to document the deficiency.

This is a Severity Level IV violation (Supplement 1)(285/97007-02) l Pursuant to the provisions of 10 CFR 2.201, Omaha Public Power District is hereby i required to submit a written statement or explanation to the U.S. Nuclear Regulatory Commission, ATTN: Document Control Desk, Washington, D.C. 20555 with a copy to the Regional Administrator, Region IV,611 Ryan Plaza D."e, Suite 400, Arlington, Texas 76011, and a copy to the NRC Resident inspector at the facility that is the subject of this  ;

Notice, within 30 days of the date of the letter transmitting this Notice of Violation "

(Notice). This reply should be clearly marked as a " Reply to a Notice of Violation" and should include for each violation: (1) the reason for the violation, or, if contested, the basis for disputing the violation, (2) the corrective steps that have been taken and the ,

results achieved, (3) the corrective steps that will be taken to avoid further violations, and l

(4) the date when full compliance will be achieved. Your response may reference or i include previous docketed correspondence, if the correspondence adequately addresses the required response. If an adequate reply is not received within the time specified in this Notice, an order or a Demand for Information may be issued as to why the license should not be modified, suspended, or revoked, or why such other action as may be proper should not be taken. Where good cause is shown, consideration will be given to extending the response time.

Because your response will be placed in the NRC Public Document Room (PDR), to the extent possible, it should not include any personal privacy, proprietary, or safeguards information so that it can be placed in the PDR without redaction. However, if you find it necessary to include such information, you should clearly indicate the specific information that you desire not to be placed in the PDR, and provide the legal basis to support your request for withholding the information from the public.
