During an NRC inspection conducted on May 5-9, 1997, one violation of NRC requirements was identified. In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," NUREG-1600, the violation is listed below:
10 CFR Part 50, Appendix B, Criterion V, states, in part, "... activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances... Instructions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance critoria for determining that important activities have been sat 6sfactorily accomplished."

Contrary to the above, on May 6,1997, the NRC inspectors discovered that there were no instructions or procedures'to ensure that alllicensed operators, who were required to wear corrective lenses as a condition of their individual licenses, had corrective lenses of the appropriate type available should these individuals be 1

required to wear self-contained breathing apparatus while performing licensed duties.

should include for each violation: (1) the reason for the violation, or, if contested, the l
l basis for disputing the violation, (2) the corrective steps that have been taken and the l

(4) the date when full compliance will be achieved. Your response may reference or 1
include previous docketed correspondence, if the correspondence adequately addresses the l
. required response. If an adequate reply is not received within the time specified in this Notice, an order or a Demand for Information may be issued as to why the license should j
not be modified, suspended, or revoked, or why such other action as may be proper should I
not be taken. Where good cause is shown, consideration will be given to extending the response time.
