ap APPENDIX A NOTICE OF VIOLATION Duquesne Light Company Docket No. 50-412 Beaver Valley Power Station, Unit 2 License No..CPPR-105 As a result of the inspection conducted on January 17 - Febr_uary 28, 1987, and in accordance with the " General Statement of Policy and Procedure'for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1985), the following violation was. identified:

10 CFR 50, Appendix B, Criterion V requires activities affecting quality to be prescribed by documented instructions, procedures or drawings which include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished.

10 CFR 50, Appendix B, Criterion X, Inspection, requires that a program for inspection of activities affecting quality shall be established and executed to verify conformance with documented instructions, procedures and drawings for accomplishing the activity.

Contrary to the above, as of February 3, 1987, Site Quality Control Inspection Plan 7.4.1 did not include appropriate quantitative or qualitative acceptance criteria for inspection of HVAC equipment supports. Specifically, the accept-ance criteria for such supports were not sufficiently detailed in that they merely required verification that structural steel supports were acceptable.

Inspection reports for both Diesel Generator Building Secondary Supply Air Fans (2HVD*FN271A and B) stated that inspection attribute ME-MI-027 (verify structural steel supports are acceptable) was satisfactory. However, these supports were unsatisfactory since each support had a missing bolt as required by drawings SM-7 and RS-29C. Reinspections of other HVAC equipment supports revealed other bolting nonconformances such as incorrect bolting material.

This is a Severity Level IV Violation (Supplement II).

Pursuant to the provisions of 10 CFR 2.201, Duquesne Light Company is hereby re-quired to submit to this office within thirty days of the date of the letter which transmitted this Notice, a written statement or explanation in reply, including (1) the corrected steps which have been taken and the results achieved; (2) cor-rective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending this response time.
