APPENDIX A NOTICE OF VIOLATION Duquesne Light Company Licwnse No. OPR-66 Beaver Valley 1 Docket No. 50-334 As a result of the inspections conducted on June 9-13, 1986 and May 9-13, 1988, and in accordance with the "General Statement of Policy and Procedura for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (Enforcement Policy) (1988),

the following violation was idertified:

10 CFR 50.49 (f) and (j), respectively, require that (1) each item of electric equipment important to safety shall be qualified by testing and/or analysis of identical or similar equipment, and the qualification based on similarity shall includo a supporting analysis to show that equipment to be qualified is acceptable; and, (2) a record of the qualifi-cation shall be maintained in an auditable form to permit verification that each item of electrical equipment important to safoty is qualified and that the equipment meets the specified performance requirements under postulated environmental ennditions.

Contrary to the above,

1. on June 12, 1986, the NRC inspectors determined that the qualification of the Victoreen High Range Radiation Monitor (HRRM) cable assembly was not established. There was inadequate test or analytical informa-tion to demonstrate that these instruments would satisfy performance requirements as a result of potentially high thermal-induced leakage current in the cable assembly during accident conditions. The HRRMs are provided to monitor variables specified in Regulatory Guide 1.97 and are therefore defined as electrical equips.ont important to safety by 10 CFR 50.49 (b)(3). Two HRRMs were identified with this deficiency.
2. on May 12, 1988, the inspector found an unidentified, unqualified wire jumper in MOV-RW-1050. This MOV was recently added to the licensee's environmental qualification master list to satisfy the guidance of Reg. Guide 1.97 and is defined as electrical equipment important to safety by 10 CFR 50.49(b)(3). In followup inspections of MOVs conducted by the licensee between May 12 and June 11, 1988, the licensee identified an additional 20 MOVs, that had been added to the EQ master list to satisfy Regulatory Guide 1.97 guidance with unidentifiable, unqualified jumper wire. In addition on June 6, 1988, during a licensee reinspection of previously inspected MOVs on the EQ master list, the licensee identified 11 MOVs in the charging and safety injection systems with unqualified wire.

These items are a Severity Level Ik violation (Supplement I).
