4 APPENDIX A NOTICE OF VIOLATION Duquesne Light Company Docket No. 50-412 Beaver Valley Power Station, Unit 2 License No. CPPR-105 As a result of the inspection conducted on December 8 - 11, 1986, and in accordance with the NRC Enforcement Policy (10 CFR 2, Appendix C), the follow-ing violation was identified:

10 CFR 50, Appendix B, Criterion V requires that: " Activities affecting quality shall be prescribed by documented instructions -- and shall be accomplished in accordance with these instructions- ."

Field Construction Procedure No. FCP-431, Section 5.2.10 states that:

" Cut edges, sharp corners and holes shall be deburred to provide smooth surfaces which will not damage cables." ,

Contrary to the above, on December 10, 1986 Cable No. 2SISA0H301 in the low head safety injection pump No. 2 SIS *21A housing was identified by the NRC inspector that had been damaged at the splice junction by the sharp edges of the fabricated stainless steel motor housing base plate.

Pursuant to the provisions of 10 CFR 2.201, Duquesne Light Company is hereby required to submit to this office within thirty days of the date of the letter which transmitted this Notice, a written statement or explanation in reply, including: (1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending this response time.
