APPENDIX A NOTICE OF VIOLATION Duquesne Light Company Docket No. 50-412 Beaver Valley Power Station, Unit No. 2 License No. CPPR-105 During an NRC inspection conducted on Septer.ber 30 - October 3, 1986, two violations of NRC requirements were identified. The violations involved system changes not properly reflected in preoperational tests and failure to assure that all prerequisites are satisfied prior to performance of testing.

In accordance with the " General Statement of Policy and Procedure for NRC En-forcement Actions," 10 CFR 2, Appendix C (Enforcement Policy 1986), the fol-lowing violations were identified:

10 CFR 50, Appendix B, Criterion VI requires in part that " Instructions, procedures, and drawings, including changes thereto, affecting quality are reviewed for adequacy and are distributed and used at the locations where the prescribed activity is performed."

Contrary to the above, as initially identified on August 22, 1986, during performance of preoperational test P0-2.07.02 " Boric Acid System Test,"

changes made to the system in E&DCR 0-0117-802 were not properly reflected in the preoperational test.

This violation was a followup to an item initially identified during an inspection conducted on August 18-22, 1986. A contributing cause to this violation was a general lack of knowledge by test and operations personnel in the drawing and document control systpm.

10 CFR 50, Appendix B, Criterion XI, requires in part, that " Test pro-cedures shall include provisions for assuring that all prerequisites for the given test have been met......"

l Contrary to the above, as identified on October 1, 1986, test personnel involved with the preoperational test P0.2.07.0.3," Chemical Volume Con-trol System" failed to check prerequisites prior to the test for the system portion of valve No.137 and, again, for the system portion of l

valve No. 4608.

l This is a Severity Level V Violation.
