1 APPENDIX A NOTICE OF VIOLATION Duquesne Light Company Docket Nos. 50-334; 50-412 {

Beaver Valley Power Station License Nos. DPR-66; NPF-73 Units 1 and 2 As a result of the inspection conducted on May 23 - July 7,1989, and in ac-cordance with the " General Statement of Policy and Procedure for NRC Enforce-ment Actions," 10 CFR Part 2, Appendix C, the following violation was iden-tified:

10 CFR Part 50, Appendix B, Criterion XII requires that measures shall be established to assure that tools, gauges, instruments, and other measuring and testing devices used in activities affecting quality are properly controlled.

Section 2 of the Instrument and Control Administrative Manual, Control and Calibrations of Measuring and Test Equipmeat, which requires that past due devices be marised and segregated from calibrated devices, and that the Daily ,

Use and Issue Log be completed each day for instruments in use implements this '

requirement.

Contrary to the above, the following two examples represent licensee failures to properly control tools, gauges, instruments and other measuring and test devices used in activities affecting quality.

(1) Approximately 90 measuring and test devices were past due for calibration and were not properly marked and segregated from calibrated measuring and test devices as required by procedure (Section 2 of the instrument and Control Manual,Section VI.F.12).

(2) Multiple examples (about 100) of the failure to properly use the Daily Use and Issue Log as required by procedure (Section 2 of the Instrument and Control Manual,Section VI.F.) were identified.

This is a Severity Level IV Violation (Supplement I).

Pursuant to the provisions of 10 CFR 2.201, Duquesne Light Company is hereby  !

required to submit to this office within thirty days of the date of the letter which transmitted this Notice, a written statement or explanation in reply, including: (1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending this response time.
