t APPENDIX A NOTICE OF VIOLATION .

Duquesne Light Company Docket No. 50-334 Beavcr Valley Power Station,. Unit 1 License No. DPR-66 As a result of the inspection conducted on May 9-13, 1988, and in accord'ance with the "Gene-ral Statement of Policy and Procedure for NRC Enforcement '

Actions," 10 CFR Part 2, Appendix C (Enforcement Policy) (1986), the following violation was identified:

Technical Specification 6.11 states that "procedures for personnel radiation protection shall be prepared consistent with the requirements of 10 CFR Part 20 and shall be approved, maintained and adhered to for all operations involving personnel radiation exposure."

Radiaticn Protection Procedure No. 7.3. requires, in part, in Section 3.4.5.1' that if the field evaluation of an air sample filter paper indicatos greater than 500 net counts per minute (ncpm), Radcon supervision and/or the Nuclear Shift supervisor are to be notified. This notification is necessary to determine if there is a need for immediate corrective actions (e.g. , evacuation).

Contrary to the above, at about 8:40 p.m. on February 7, 1988, an air sample filter paper, collected during hydrolazing of the Unit 1 Fuel Transfer Canal, was found b) field evaluation to indicate 2400 ncpm and supervision was not notified to allow them to determine.if immediate corrective action!; were needed.

This i s a Severity Level IV vio'ation (Supplement IV).

Pursuant to the provisions of 10 CFR 2.201, Duquesne Light Company is hereby required to submit to this office within thirty days of the date of the letter i which transmitted this Notice, a written statement or explanation in reply, including: (1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved. Where good cause is shown, con sideration will be given to extending this response time.
