Inspection 50-412/87-02
This refers to the routine resident safety inspection by Messrs. J. Beall and
L. Prividy, and Ms. A. Asars on January 17 to February 28, 1987, at the Beaver
Valley Power Station, Unit 2, Shippingport, Pennsylvania.
The inspection consisted
of document reviews, interviews and observation of construction and preoperational
test activities.
The findings were discussed with Mr. R. Swiderski of your staff.
Areas examined during this inspection are described in the NRC Region I Inspection
Report which is enclosed with this letter.
Within these areas, the inspectiCn con-
sisted of selective examinations of procedures and representative records, inter-
views with personnel, and observations by the inspectors.
Based on the restits of this inspection, it appears that certain of your activities
were not conducted in full compliance with NRC requirements, as set forth in the
Notice of Violation enclosed herewith as Appendix A.
The violation has been cate-
gorized by severity level in accordance with the " General Statement of Policy and
Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1985).
You are
. required to respond to the enclosed Notice, and in preparing your response, you
should follow the instructions specified therein.
In addition, as discussed in Detail 11 of the attached inspection report, an ap-
parent weakness in the Duquesne Light Company Quality Assurance Program was ob-
served.
The enclosed Violation is the third recent example in which deficient
hardware was accepted due to inadequate guidance provided to Quality Control in-
spectors.
We are concerned that the repetitive nature of these findings may be
indicative of a programmatic weakness in the process which produces the guidance
used for accepting safety related components.
You are requested to address this
apparent weakness in your response to the enclosed Notice of Violation.
The responses directed by this letter and the accompanying Notice are not subject
to the clearance procedures of the Office of Management and Budget as required by
the Paperwork Reduction Act of 1980, PL 96-511.
