APPENDIX A NOTICE OF VIOLATION Duquesne Light Company Docket No. 50-334 Beaver Valley Power Station, Unit 1 License No. DPR-r6 As a result of the inspection conducted on January 25 - 29, 1988, and in accordance with the NRC Enforcement Policy (10 CFR 2, Appendix C), the following violation was identified.

A. Technical Specification 6.11 states that "procedures for personnel radiation protection shall be prepared consistent with the requirements of 10 CFR Part 20 and shall be approved, maintained and adhered to for all operations involving personnel radiation exposure.

Procedure RP 8.1, Radiological Work Permit, section 3.2.1.8, requires in part,that individuals sign the applicable RWP, "...signif{ing understanding and acceptance of the conditions and controls of the RWP.

Contrary to the above, on January 26, 1988, a Radiological Controls technician entered the Unit I containment while not dressed in accordance with the minimum anti-contamination clothing requirements of the applicable RWP. Specifically, the technician entered containment without a cloth hood, The controllin coveralls, or a second pair of shoecovers and gloves. required anti-contamination clothing to inc coveralls, and two pairs each of shoecovers and rubber glovas.

1his is a Severity Level IV violation (Supplement IV).

Pursuant to the provisions of 10 CFR 2.201, Duquesne Light Company is hereby required to submit to this office within thirty days of the date of the letter which transmitted this Notice, a written statement or explanation in reply, including: 1) the corrective steps which have been taken and the results achieved; corrective steps which will be taken to avoid further violations; and (3) the(2 ate when full compliance will be achieved. Where good cause is shown, consideration will be given to extending this response time.
