APPENDIX A NOTICE OF VIOLATION Duquesne Light Company Docket No.

50-334 Post Office Box 4 License No. DPR-66 Shippingport, Pennsylvania 15077 As a result of the inspection conducted on March 21-April 1, 1988 and in accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions", 10 CFR Part 2, Appendix C (Enforcement Policy, 1987),

the following violation was identified.

Section 6.8.1 of Beaver Valley Power Station Unit No. 1 Technical Specifica-tions states in part:

"Written procedures shall be established implemented and maintained covering the applicable procedures recommended in Appendix A of Regulatory Guide 1.33 Revision 2, February 1987.

Appendix A of Regulatory Guide 1.33 includes procedures for equipment control (1.c) and performing maintenance (9).

Two examples of the licensee's activities not meeting the regulatory require-ment were:

(1) The licensee's Quality Assurance Procedure Number OP-11, Revision 7; paragraph 11.4.2 requires in part:

.... detailed implementating procedures shall provide means for assuring.... that the acceptability of systems and equipment is known throughout operation....".

Contrary to the above on March 4, 1988 the acceptability of the emergency diesel generator starting air system and associated equipment was not known during operation in that seismic supports, for the air start accumulator, adjacent to valves 10A-138 and IDA-140, were degraded due to loose bolts.

This caused the emergency diesel generator number 2 to be declared inoperable.

(2) The licensee's preventive maintenance procedure (PMP) No. 1-44VS-VNT-1E, Revision 3, Table 1, requires the ventilation system dampers VS-D-22-2C and VS-D-22-20 and ventilation system fans VS-F-22A and VS-F-228 to receive preventive maintenance at a frequency of 12 months.

Contrary to the above, as of March 30, 1988, there was no objective evidence to assure that the dampers VS-D-22-2C and VS-D-22-20 and the fans and VS-F-228 received preventive maintenance at a frequency of 12 months since May 1986.

In addition, inlet dampers VS-D-22-2A&B and VS-D-22-1A&B were not included in PMP No. 1-44VS-VNT-1E.

The above two examples collectively constitute a Severity Level IV violation (Supplement I).

Pursuant to the provisions of 10 CFR 2.201, Duquesne Light is hereby required to submit to this office within thirty days of the date of the letter which transmitted this Notice, a written statement or explanation in reply, including: (1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved. Wnere good cause is shown, consideration will be given to extending this response time.
