11. SUMFARY OF ISSUE The problem as defined by TVA was that, heat numbers were placed on approx-raately five p!etes on the containtrent vestel without the knowledge of Quality Lontrol. The heat nun.bers were added when it was ciscovered that there were l no heat numbers on the plates. l

!!!. EVALUATION The review by the Erployee Concern Task Group (ECTG) evaluator of the NSRS report indicated that the en.ployee concern was not substartiated. The records incicated that the heat nur.bers for the five plate attached to the centainment  !

Ti A Element Report 40512-SQN concluded that there were no requirements to  !

1 riace or verify heat nur.bers on the plates at fabrication or installatten.

The eierent report also concludes that one of the five plates attached to the  ;

containment liner was not AS1h A516 Grade 60 or 70 as specified, but ASTM A36. l The element report state that this plate was support pad mark number 125, i

The eier.ent report indicates that the following corrective actions will be taken:

1. Metallurgical test will be performed on the support pad in question to det' rmine the type and grade of the r.cterial.

I 2. Remove arid replace with the cor, rect material, if incorrect.

The corrective actions were complated on July 28, 1988 and the RIMS documenta-2 tien nurter is T25850728942.

IV. CONCLUSION i

i The ARC staff concludes that TVA's investigation of the concern was adequate and their resolution of the concern, as described in Element Report MC-40512-SON Revision 4 is acceptable.
