0YSTER CREEK NUCLEAR GENERATING STATION Forked River, New Jersey 08731 Special Report 86-15 Report Date December!16,1986 Occurrence Date November 17, 1986 Identification of Occurrence Failure to restore degraded fire barrier penetration seals to functional status within 7 days from the time of discovery. This event is considered to be reportable as defined in Technical Specifications, paragraph 3.12.E.3.

Description of Occurrence On November 10, 1986, during the installation activities of conduit fire barrier material (Thermo-Lag) for Appendix R requirements, two conduit penetrations through the floor of the 4160 volt switchgear ID vault and penetrations through the floor of the motor generator set room were discovered to be degraded. Specifically, the penetration seal configurations only rendered a one hour fire rating in lieu of a three hour fire rating as required by the Oyster Creek Fire Hazards Analysis. Due to the existing fire loading in the area of the degraded seals being below a one hour fire severity, repair activities to upgrade the seals to a three hour fire rating l

was not given top priority. This was based on the rating 9f the degraded seals still being adequate to protect the areas from the hazards present.

Due to the above consideration, the penetration seals were not restored to a functional condition within 7 days as required by Technical Specifications paragraph 3.12.E.3.

Corrective Actions L An hourly fire watch patrol was established within one hour of discovering the degraded fire barrier seals as required by Technical Specifications paragraph 3.12.E.2. The hourly fire watch patrol will continue until the seals are restored to functional status which presently is scheduled to be completed by l February 28, 1987.
