SPECIAL REPORT 90-04 Report Date:

September. 21, 1990 Identification of Occurrence:
A non-functional fire barrier door was not repaired within seven (7) days as required by Technical Specification 3.12.E.
Descriotion of Occurrence 1
During a routine weekly surveillance of the fire barrier doors, the door between fire zones TB-FZ-11C and TB-FA-26 was identified as not meeting the acceptance criteria. The door was declared non-functional due to it not being i
able to latch under its own power.
' Analysis of occurrence:
Upon discovery of the non-functional fire barrier door, a work request was submitted to repair the fire barrier door within.seven (7) days.
Due to constraints imposed by increased maintenance activities of a higher priority, it was not possible to accomplish this repair within seven (7) days as specified. Since the required fire watches had been established, the safety significance of this event is minimal.
Corrective Action Upon discovering the non-functional fire barrier door, within one hour a fire watch was. established in accordance with the technical specification.
