Non-functional fira diesel pump 12 was not restored to functional status within seven (7) days as required by technical specification 3.12.B.
Descr_ipfion of Occurrencel During a functional test, fire diesel pump 1-2 failed to meet the required acceptance criteria. The pump failed to obtain the requirement of 156 psig at a 2000 gpm-During testing at 2006 gpm the pump could only attain 78 psig.
Analysis of Occurrence:
Upon failing the functional test, a work request was submitted to restore the pump to functional status.
The Plant Maintenance Department removed and disassembled the pump discovering that a cloth rag had become lodged inside the pump.
It was determined that a gap between the basket strainer and the pump suction bell was large enough to allow this rag to enter the pump. During this time period, the 1-1 diesel fire pump and the redundant fire pump were available, if needed.
Thus the safety significance of this event was minimal.
Corrective Action:
Fire Diesel pump 12 was rebuilt and reinste11cd on 12/8/91. A corrective change was made to the basket strainer to reduce the gap at the pump suction bell.
After work was completed, an in-service test and a functional survelliance were performed on 12/11/91.
The pump was declared operable and returned to service on 12/12/91.
