July 31,1991 Identification of Occurrence:

A non-functional fire barrier door was not repaired within seven days as required by technical specification 3.12.E.

Qescription of Occurrence-On June 24, 1991, at 1130 hours0.0131 days <br />0.314 hours <br />0.00187 weeks <br />4.29965e-4 months <br />, the fire barrier door that connects the emergency diesel generator vaults (fire zones DG-FA-15 and DG-FA-17) was declared non-functional. An hourly fire watch patrol was established as required by tech spec 3.12.E.2 until repairs were made.

Analysis of Occurrence The door was declared non-functional due to a broken door closure mechanism.

During the entire period that the door was non-functional, an hourly fire watch was in place and the door was kept closed. The safety significance of this event is minimal.

Corrective Action On July 4,1991, the door closure mechanism was repaired. The door was then declared functional and the fire watch was secured.
