This report is overdue because a deviation report was not written when a fire barrier door was declared non-functional. To ensure that reporting requirements are satisfied in the future, this report will be made required reading for all operations department personnel.

July 2, 1991 Identification of Occurrence:

A non-functional fire barrier door was not repaired within seven days as required by Technical Specification 3.12.E.

Description of Occurrenc_q A review of the fire watch patrol list for the period April 20, 1901 through July 2,1991 identified that the door between fire zones 08-FZ-10A and TB-FZ-118 had been non-functional for greater than seven days. The door was not able to latch under its own power.

Analysis of Occurrence The subject door is affected by ventilation in the area; it either will not close against air flow or will slam shut causing increased maintenance. Due to changing area ventilation, the operable status of this door changes frequently.

During the entire period that the dcc was inoperable, an hourly fire watch was established. The safety significance of this event is minimal.

Corrective Action On July 2,1991, the door was declared operable and the fire watch was secured.

To address the recurring problem with this door, GPUN is evaluating alternate door closing mechanisms.
