On Behalf of our clients, Mr. Terry Howard and Ms. Sharon Morella, and pursuant to both the Freedom of Information Act (5 U.S.C.

E 552) amd the Privacy Ar c ( 5 U.S.C. 8 552a), we request copics of all internal notes, memoranda, drafts and/or other records prepared at Region III by all investigators involved in the investigation of our clients allegations at the La Salle Nuclear Power Plant.* We specifically request all inspection Dvaluation Forms prepared at region III by all investigators involved in said investigation at LaSalle Nuclear Power Plant.* nWe also request all internal memorandum, engineering analysis, notes, telephone logs, tapes, diaries and/or other records concerning problems that have been discovered at the LaSalle Nuclear Power Plant as a result of said investigation.

If any material covered by this request has been destroyed and/or removed, please provide all surrounding documentation, including but not limited to a description of the action (s) taken, relevant date(s), and justification for the action (s).

Mr. Howard and Ms. Morella request that the fees be waived, because

" finding the information can be considered as primarily benefitting the general public." 5 U.S.C.8552 (a) (4) (A). The Government Accountability Project is a non-profit, nonpartisan public interest organization concerned with honest and open governnent. Through legal representations, advice, national conferences, films, public outreach and publications the Project promotes whistleblowers as agents of accountable government. We are requesting the above information on behalf of our clients for a monitoring project on the adequacy of the NRC's efforts to protect public safety and health at nuclear power plants.

For any documents or portions of documents that you deny due to a specific exemption, please provide an index itemizing and describing the documents or portions of documents withheld. .The index should provide a detailed justification of your grounds for claiming each exemption, explaining why each exemption is relevant

  • Excluding the Clinton Nuclear Power Plant and the Midland Nuclear Power Plant but including any O.I. and I.E. investigations.
