During a startup of the 1A. River Water Pump (WR-P-1A) -

on June 20, 1991, the pump failed. An investigation of the failure was initiated and revealed a mechanical failure of the shaft coupling. A root-cause analysis was initiated to determine the failure mode.

Independent laboratory testing results received from l Lehigh U.tiversity on September 25, 1991, attributed the coupling failure from the 1A pump to-embrittlement of the 410 S.S. material due to improper tempering temperatures and the potential presence __ of tramp elements. Impact tests on the failed coupling showed-  ;

an energy absorption of six foot-pounds-in a 7MM X 10MM charpy V-notch sample at room temperature.- This was ,

the lowest value exhibited by - the four couplings from-the suspect heat and of_the three heats of couplings tested.

A review of pump oporating and test records showed that vibration levols were trending upwards prior to the fa13ure. This may be indicativo of a condition which led to the coupling failuro. (floto: Unit i Technical ,

water pumps to be oporeblo. ) Vondor analysib indicated a worn bearing and/or pump shaf t misalignment led to a condition which rusuited in coupling fa d luro.

l The failed coupling was traced to a 1977 purchaso order (C003708) that purchased a total of 12 couplings from Dyron Jackson. All 12 couplings woro from tha name heat natch (llT / 8 213 3 6 ) and shop route slip ,

(R/ S164 34 0) . In addition, a purchase order i t. 1976 '

(co02470), that purchased a total of 18 couplings, contained one coupling with that same heat number. A so_ arch for the suspect couplings and their installation history was conducted. It- was enneluded that coupling from the suspect heat cou3d also be installed-in the IB

& 10 river water pumps.

The 1A rivnr water pump has boon refurbished with newly _

l purchased and lualified safety-related couplings that were tested to .4uro acceptability and verified not to be from the ausset batch, one of the now couplings was testod for hardness and impact resistance to cnsure its machanical proporties mout vendor requirements and the- impact resistanco exceed that of the failed coupling.

The failed coupling will be shipped to. Brookhaven Labs-for_ further examination as requested by 11R C staff personnel. Further ovaluations and discussions with '

the vendor are ongoing to obtain additional information on the coupling material.

(v) Dato on Which Doroot Identified:

The failure of the coupling in the 1A pump occurred on June 20, -1491. Pump disassembly' work was initiated on June 21, 1991. Dolays woro experienced due.to an availability of parts. Tho 1A pump repairs woro completed on August 29,_1991. Pump performance testing on September 4, 1991 failed due to low head at' required flow. The pump was disassembled on September 12, 1991.

P The lab analysis results on the failed coupling were roccived;on September-25, 1991. The documentation / traceability search for the suspect ,

couplings was completed - on October 17, 1991.- A BVPS Onsite Safety Committoo mooting was conducted on i October 24, 1991 and concluded the issue was reportablo under 10 CFR Part 21.
