! Duquesne Light Company initiahy submitted a 10 CFR 21 Report on November

  • 1,1991 regarding the Beaver Valley Power Station (ilVPS) Unit I river water pump coupling failure. The pumps were supplied by 11WIP,13yron Jackson Pump Division.

A folinw up report was submitted on July 7,1992, which concluded the results of the matenni analysis. The cause of the failure was the lack of material foughness.

. Operating conditions combined with low toughness may have hastened the initial failure, but the analysis confirmed the cause to be material related.

During maintenance and overhaul of. the lleaver valley Power Station Unit 2 Service Water Pump 2SWS*P21 A, two coupling sleeves were found to contain longitud;nal cracks at corners of the key way slots. These couplings, both produced l from material heat #72337, were sent to lxhigh University Energy Research Center to confirm the material chemistry, m:chanical properties, and the failure mechanism. A third coupling, which was also produced from heat #72337, was found cracked on the Service Water Pump 2SWS*P21C and was also sent to Ixhigh for analysis.

The analysis cencluded the cause of falh.re of the couplings on the Unit 2

  • Service Water Pumps was identical to that for the Unit 1 River Water Pump Couplings, even though the design is a keyed coupling rather than threaded as in Unit 1.

Replacement couplings were screened to insure adequate toughness and t1 purchase specification was amended to include impact requirements on the coupling m erial.

q Since the vendor's material requirements do not state a minimum toughness level, the potential exists for similar coupling failures on like equipment at other facilities.
