ATTACHMENT POTENTIAL SIGNIFICANT DEFICIENCY REPORT 87-18

1. Summary During system check-out (prior to fuel load) the emergency switchgear supply fan 2HVZ*FN261A and diesel generator room supply fans 2HVD*FN270A and 271A did not start by manual actuation of their control switches on the alternate shutdown panel (ASP), as required, because the switches were spring return type and the start signal was not sealed in.
2. Immediate Action Taken E&DCR D-0183-508 has been revised to replace the control switches with maintained two-positioned switches.

Mr. D. G. Szucs, Compliance Engineer, Beaver Valley Power Station Unit No.

2, notified Mr.. D. Limroth, NRC Region I of this concern on August 3, 1987.

3. Description of Deficiency Advanced Change Notice ( ACN) D-0183 was issued September 26, 1986 to pro-vide manual control at the alternate shutdown panel (ASP) for the emer-gency switchgear supply fan 2HVZ*FN261A and the diesel generator room supply fans 2HVD*FN270A and 271A. Installation of this modification used a two-position Westinghouse OT2 type, spring return to center control switch on the ASP for manual control of this equipment. The start signal when actuated via this control switch was not sealed in and, therefore, was lost when the switch was released. As a result, the fans did not start, as required, by manual actuation at the ASP.
4. Analysis of Safety Implication Lack of ventilation in the " Orange Train" emergency switchgear area or the

" Orange Train" diesel generator room area could result in an increase in temperature above design limits. This constitutes a deficiency in plant design which, were it to have remained uncorrected, could have adversely affected the safety of operations of the plant.

5. Corrective' Action to Remedy Deficiency The control switch has been replaced on E&DCR D-0183-509 with a two-position Westinghouse OT2 type maintained control switch. This switch will remain in the start position, allowing the. fans to operate as required from the ASP.
6. Additional Report This is a final report. No additional reports are anticipated. ,
