Washington, DC 20555 NOTIFICATION OF DEFECT BEAVER VALLEY POWER STATION - UNIT 2 DUQUESNE LIGHT COMPANY Stone & Webster Engineering Corporation (SWEC), pursuant to the provisions of 10CFR21, hereby provides notification of a " defect" concerning the Beaver Valley Power Station - Unit 2, owned in part by Duquesne Light Company (DLC), and for which SWEC provided both engineering and construction services.
In accordance with the reporting requirements of Section 21.21(b)(3), the following information is submitted:

During the course of disassembling Target Rock modulating solenoid valve 2SWS*TCV101B, a wooden dowel pin was found ic place of the required stainless steel spiral pin. The spiral b

A CB 2 pin is used' to' prevent _ rotation of the disc rod which is threaded into the plunger. Rotation of the disc rod could result in an increase or decrease to valve stroke causing excessive leakage. or reduced opening. Valve 2SWS*TCV101B is-used to maintain condenser inlet water temperature between 72 F i and 86*F to maintain compressor head pressure within . design !
limits.
Malfunction of the valve would result _in a trip of the refrigeration unit which, in turn, would ' result in a loss of cooling for the Control . Room area until such time as the redundant unit is operating. Loss of the ' refrigeration unit during a LOCA, coincident with a single failure of the redundant i train, could cause the equipment environmental qualification temperature of 104 F to be exceeded in 12.5 ninutes.
Additionally,.there would not be enough time for the operator to i open the' bypass valve in a time frame consistent with the <
guidance set forth in ANSI /ANS 58.8-1984. This represents a l major reduction in the degree of protection provided to the l public health and safety and could create a substantial safety hazard.

Beaver Valley Unit 2 is, to date, the only project for which SWEC has or had applicable engineering, design or procurement responsibility determined to have this defect. Should this
.information change, this report will be amended accordingly.

The valve was reassembled using the proper spiral pin. A duplicate valve, manufactured at the same time as the deficient valve and assembled by the same technician, was inspected and i no improper parts were found, viii. Advice related to the defect that has been given to purchasers or licensees:
No other action apart from that specified above. Correspondence j with the vendor indicated that this was the only case reported in over 5,000 solenoid valves which had been shipped since 1972.
