Duquesne ...aht notified ME/P8D of failures with the contactor used on the diesel sonorator immersion heater (EMD #8455508).

To date four (4) contactore have experienced failures. ME/PSD 4 returned two (2) of the failed contactors to Outler-Hamar l (manufacturer for END) to determine the cause of the failure.

The failure report from Outler-Hamar indicated that dimensional probleme existed in the contacts which caused the failures.

Cutler-Hammar has made changes in the dimensions to correct the problem. In conversation with Mr. Charles Wade (602/782-1565, ext. 334) at Cutler-Hammer, the following was amoertained:

1. Cutler-Hammar does not know how far back the problems )

exist in the contaotors. The only date they can give, is that the problems were fixed in the 20th week of 1989 (roughly week of 15 May 1989). ,

2. Mr. Wade does not know if EMD has been notified. He is currently investigating this.

3. The problems exist not only in the EMD contactorm, but mino in all of the NEMA size 1, 2, and S contactors.

4. The probles first occurred in 1988.

The report from Cutler-Hamar le attached.

We are still investigating this with Cutler-Bammar to determine more specifies about the problem and when it first occurred.  !

Currently, all contactors with date code before 5/22/89 are suspect. A formal report will be issued next week as more data becomem available.
