. Epecial Report 91-03 j Report Dates March 28,1991 Occurreneo Date:

2/22/91 Identification of_Occurreneg I Inoperable fire hose station 33 on El. 23'-6" of the Reactor Duilding was not restored to operable status within 14 days au required by Technical ,

specification 3.12.D.3.

Descriotion of Occurreness The 2 1/2" angle valve supplying hose station 33 was tagged out of service on j

- 2/22/91 to allow work to proceed with the installation of a temporary sprinkler i system.

Analysis of occurrence: '

A temporary automatic sprinkler system being installed for ptotection of the drywell work area uses the stand pipe supporting hose station 33 for its water !

supply. This required closing the 2 1/2" angle valve which controls the home i station while the sprinkler system piping was installed preventing use of the hose station.

Eqrrective Actions The requirement of Technical specification 3.12.D.2 was established within 24 hours2.777778e-4 days
0.00667 hours
3.968254e-5 weeks
9.132e-6 months
when additional hose was provided at hoes station 29 approximately 35 feet northwest of hose station 33.

The valve was reopened on March 12, 1991_ restoring hose station 33 to operability. Concurrent with this action, the drywell-automatic sprinkler system was completed and placed in service.
