MEETING MINUTES: Internal IMC 0350 Oversight Panel Meeting Davis-Besse Nuclear Power Station DATE: December 3, 2002 TIME: 1:00 p.m. Central ATTENDEES:

1. Plant Status and Inspector Insights The Panel discussed the status of the draft Task Interface Agreement (TIA) submitted to NRR related to potentially nonconservative Technical Specification (TS) requirements associated with minimum trisodium phosphate dodecahydrate (TSP) inventory and maximum allowable unidentified leakage.

S. Thomas briefed the Panel on current site activities:

C Reactor coolant pump overhaul and emergency core cooling system sump work were underway. Drain-down of the reactor coolant system was completed to support refurbishment of 74 ECCS valves and to support the reactor coolant pump work.

C An update of major milestones was provided:

Begin core reload: 1/13/03 Mode 5: 1/18/03 ILRT: 1/25/03 Mode 4: 1/29/03 Mode 3: 1/31/03 Cooldown to Mode 4/5 (for Inspection): 2/6/03 NRC Restart Approval: 2/10/03 Mode 3: 2/17/03 Mode 2: 2/18/03 Mode 1: 2/19/03 100% Power: 2/21/03 Scott mentioned that the above dates do not incorporate the the decay heat pit design delays which could easily push all the Mode 4 dependent dates out at least a month.

C There was an emergent issue regarding the licensees discovery of a crack in at least one of four thermal sleeves that extends into the Reactor Coolant Pump

discharge piping. Possible causes were believed to be thermal fatigue or flow induced vibration or some combination. The licensee intends to replace the thermal sleeves. The licensee discovered this issue while performing inspections during deep drain valve work. A Region III DRS inspector was asked to assist in the followup of this issue.

C Scott reported that the licensee had a significant number of condition reports classified as "significant condition adverse to quality" that were past due for completion of their root cause evaluations. The licensees process for requesting extensions for completing the evaluations appears weak and was not followed in many instances.

C Scott reported on the status of system reviews. The licensee was supposed to complete all 31 system reviews (discovery) by December 6, 2002. The present status of the review were: six complete, 17 having been presented to the Engineering Assessment Board (EAB) for comments, and 8 that were not completed to the stage of being reviewed by the EAB. The plan for the eight remaining systems was to revise the review process, to not require EAB review, and to simplify the validation process (the actual details of the simplification still had not been firmed up). The Panel was planning to do further followup on the licensees rational and ramifications for not sending the systems through the EAB.

Review of action items (panel)

The panel reviewed the list of open action items, with discussions and actions as follows:

Item 26 was closed. Letter issued 11/27/02.

Item 72 was closed.

Item 73 was discussed. NRR desires to have a revised IMC 0350 procedure issued by the end of this calendar year. NRR members of the panel took action to discuss the procedure with the procedure owners in NRR.

Item 74 was discussed. The subject matrix document remains incomplete/unofficial, but progress on completing it continues.

Item 85 was discussed. Region III received a letter dated 11/5/02 from J. Zwolinski related to the Davis-Besse Reactor Pressure Vessel Head. The letter regarding the release of the Davis-Besse head stated that "With regard to the Phase 3 sample....we will forward to you, at a future date, if any further actions are required." A followup email was received from J. Zwolinski on 11/13/02 stating that the aforementioned phrase was not intended to mean that the licensee hold the Phase 3 samples any longer than they would otherwise hold them or that they receive NRC permission prior to disposing of the samples. In order to avoid confusion when the "official" letter is sent to the licensee from Region III, J. Zwolinski requested that the sentence be re-worded to state that "The Phase 3 samples are also removed from the quarantine provisions." Region III will ensure that the letter to the licensee reflects the re-wording.

Item 88 was discussed. TIA 200201, "Response to Request for Technical Assistance -

Risk Assessment of Davis-Besse Reactor Head Degradation (ADAMS Accession No. ML023310723) was the current version of the TIA. A conference call to discuss the TIA was scheduled for later today.

Item 89 was closed. The requested input was received and incorporated into USNRC Inspection Report 50-346/02-07, issued on November 29, 2002.

Item 107 was discussed. J. Grobe at a prior meeting presented a draft paper discussing the issue of adding "incomplete and inaccurate information" for records and communications accuracy to the restart checklist. The Panel considered the points made and a decision on adding the item was tabled for further consideration.

Item 112 was closed. Future ROP dates are: 12/9, 1/13, 2/10, 3/10.

Item 114 was closed. No new news on a potential site visit by Brian Sheron.

Item 116 was closed. This item will be tracked by Item 107.

Item 119 was closed. The specific review was completed and an 11/19 email was sent discussing the Panels review of the AIT followup report.

Item 121 was discussed. The Panel discussed getting input from NRR via a memo or other following the 11/26 public meeting with the licensee regarding NRRs assessment of the appropriateness or reasonableness of the licensees plans for inspecting the bottom head of the reactor vessel.

Item 124 was closed. 11/19 email sent discussing the DBOP review of the AIT followup report (50-346/02-08)

New Issues for Inclusion into the Restart Action Matrix The Panel discussed the work on a draft procedure for adding/removing items from the RAM. D. Passehl added Item C-25 to the RAM regarding the licensees plans for perform a test during normal operating pressure and temperature (NOP/NOT) conditions to evaluate potential leakage from nozzles on the reactor bottom head. A public meeting was held in Headquarters on 11/26 on this topic.

Licensing Issues/Actions (DLPM) - status sheet The status of Davis-Besse licensing activities and ticket tracking was presented by J.

Hopkins. The licensee Code exemption requests, needed for use of the Midland reactor vessel head at Davis-Besse, have been reviewed by NRR technical staff and safety evaluations have been prepared. NRR is evaluating recent allegations of non-conforming conditions with the new head before signing out the Safety Evaluations.
