Hatch Docket No.: 50-366 Narrative Report for LER 50-366/1982-055 On May 24, 1982, with Unit Two in startup mode, QC observation as per procedure HNP-0-821 " Quality Control Work Inspection Program", of the perfonnance of procedures HNP-2-3604 "GE H2 Analyzer Calibration" and HNP-2-5223 "GE H2 Analyzer " discovered that the Offgas 2N62-N009A and N009B high hydrogen alarm setpoints were not specified or required to be verified by procedure. Unit One procedure number HNP-1-3604 "GE H2 Analyzer Calibration" specifies a one percent high hydrogen alarm setpoint; thus there is no correlation to Unit One. Section 2.1.3.h.

of Environmental Tech. Specs. for Units One and Two require that the Offgas " Hydrogen Gas Monitoring System shall provide alarms locally and in the Control Room at a setpoint of four percent hydrogen...".

Since both Offgas Hydrogen Monitoring Loops A and B had no verified alarm settings by procedure, no redundant system was available. Alarms were visually observed to be set at one percent hydrogen. Plant opera-tion was not affected. The health and safety of the public was not affected.

This event is non-repetitive.

The cause of this event has been attributed to personnel error. Per-sonnel failed to include all necessary verifications in the check and calibration procedures. Non-Conformance Report number 82-92 was submitted and procedures will be submitted to PRB for revision by June 25, 1982.
