fe SUPPLEMENTAL INFORMATION FOR LICENSEE EVENT REPORT 81-29

1. Cause Description and Analysis On November 30, 1981, at approximately 1330 hours0.0154 days <br />0.369 hours <br />0.0022 weeks <br />5.06065e-4 months <br /> with the unit at hot shutdown conditions, a leak was discovered in the Chemical and Volume Control System (CVCS). The Jeak source was located and identified as coming from Reactor Coolant Pump Seal Injection Iso-lation Valve CVC-291. CVC-291 and the associated flowpath from "A" charging pump were isolated at 1356 hours0.0157 days <br />0.377 hours <br />0.00224 weeks <br />5.15958e-4 months <br /> which terminated the leak.

Investigation revealed that the valve bonnet-to-body gasket had failed and resulted in approximately 1500 gallons of low activity water being released from the CVC System to the Auxiliary Building floor drains and subsequently to the Waste Disposal System.

This event resulted in the abnormal degradation of a system designed to contain radioactive material which is reportable pursuant to 6.9.2.b.4. The leakage was confined to the Aur.iliary Building with no release to the environment so there was no threat to the public health and safety.

2. Corrective Action As stated above, valve CVC-291 was isolated at 1356 hours0.0157 days <br />0.377 hours <br />0.00224 weeks <br />5.15958e-4 months <br />. The valve bonnet-to-body gasket was replaced, and the valve, including the flowpath from "A" charging pump, was returned to service at 2311 hours0.0267 days <br />0.642 hours <br />0.00382 weeks <br />8.793355e-4 months <br /> on November 30, 1981.
3. Cctrective Action To Prevent Recurrence A detailed inspection of the gasket and valve showed no obvious unusual degradation. This event is therefore considered an isolated failure, and no further action is necessary, i tm
