Dear Mr. O'Reilly:

' This confirms the conversation between Mr. Terry McNeill, Shift Operating Supervisor, and Mr. Al Morrongiello, at 2257 on January 16, 1982, concerning an immediately reportable event per Technical Specification 6.9.1.8e and f.

At approximately 1700 on January 16, 1982, following a reactor scram on Unit No. 2, it was determined that both loops of RHR Service Water were inoperable whfin etempting to initlate suppression pool cooling. An investigation of.

this event determined thst one loop of RHR Service Water was inoperable due to

! instrument line plugging en the suction pressure switch 2-E11-PS-1175. The j other. loop was determined to be inoperable due to a circuit breaker for its' -

pump cooler isolation valves being left in the open position earlier in the day while performing a system fresh water flush. Both loops were restored to i service and normal suppression pool cooling was established. Suppression pool

cooling temperature limits were not approached during this evolution. An investigation is currently in progress to determine the cause of the fouling
