Dear Mr. O'Reilly:

At 1119 on December 15, 1980, during routine surveillance, it was discovered that the particulate filter holder of the Unit No.1 Reactor Building Exhaust Ventilation Monitor,1-CAC-AT-1264, was not installed, thereby making the monitor inoperable. An investigation revealed that the particulate filter holder had n'ot been reinstalled during a routine weekly filter replacement at 0836 on December 11, 1980, due to a personnel error. Upon discovery, the filter holder was immediately replaced and the monitor returned to normal service. A change is being made to the plant operating procedures to ensure that this and all other similar monitors are checked for proper operation and condition each time the monitors' particulate filters are changed.

The technician responsible for replacing the filter holder has been counselled on the importance of this equipment and of the requirement to assure that any system is fully operable af ter any type of maintenance is performed on the system. All other RC&T technicians have also been counselled on this requirement.
