This cenfirms the telephone conversation at 0928 on D9 cember 31, 1981, between lir. D. C. Cooper, Shift Operating Supervisor, and fir. Jose Reyes, concerning an immediately reportable event per Technical Specification 6.9.1.8b. At 0830 on December 31, 1981, it was discovered that the required action per Technical Specification 3.3.la had not been taken on December 28, 1981, when reactor water level instrument, B21-LT-N017D-1, had been found inoperable. The required action was immediately taken and an investigation into the reason for not entering the action statement on December 28, 1981, has begun. The instrument N @
