Dear Mr. O'Reilly:

A comprehensive review and audit of documentation related to the plant Fire Protection System revealed that seventeen system LCOs had exceeded their respective time frames (of seven or fourteen days) and had not been reported to the Commission in accordance with Technical Specification 6.9.2. Fourteen of these LCOs were generated during 1982 and 1983 to permit the performance of work required for installing plant modifications in the Service Water Building. Two of these LCOs were generated in 1981 to permit the installation of fire dampers in the Diesel Generator Building, one LCO was generated in 1983 to permit the repair of an inoperable fire door in the Control Building, and one LCO was generated in 1983 due to the inoperability of the Fire Detection System in the Augmented Off-Gas Building (A0G).

The exceeding of the respective time frames of these LCOs was not reported to the Commission as a result of the following:

1. At the time, there was no established system for the daily tracking of fire barrier seal LCOs. Consequently, whenever an LCO time frame was exceeded, there was no mechanism by which to notify plant Regulatory Compliance personnel that a 30-day special report, in accordance with Technical Specification 6.9.2, was required.
2. Plant procedures did not identify which plant support group was responsible for ensuring notification of exceeding a Fire Protection System LCO time frame was made to the plant Regulatory Compliance group so that the required report would be made to the Commission. As a result, whenever an exceeded LCO time frame was discovered, it was possible that personnel identifying the event did not notify the plant Regulatory Compliance group.

4 Mr. James P. O'Reilly To help ensure timely reporting of exceeded Fire Protection System LCO time frames, the following changes have been impicmented:

1. Plant Operating Instruction 0I-04, which deals with the handling, tracking, and disposition of plant LCOs, was revised on April 8, 1983, to allow for more positive tracking of plant Fire Protection System LCOs.

In addition, it is now specifically outlined in the procedure which plant group is responsible for notifying the plant Regulatory Compliance group of an exceeded LCO time frame and how to accomplish the notification.

2. Special training was given to members of the plant Fire Protection group on April 13, 1983, to ensure their knowledge of the LCO training and handling system with respect to the plant Fire Protection System LCOs as reflected in the recently revised OI-04.
3. The plant Senior Fire Protection System Specialist will now maintain a daily running account of all active Fire Protection System fire barrier seal LCOs to help ensure those which exceed their specified time frames are reported to the plant Regulatory Compliance group so that timely reporting to the Commission may be accomplished.
4. Plant Senior Management will now review each working day the statins of all active plant and Fire Protection System LCOs. This will help ensure appropriate actions are taken to permit the cancellation of these LCOs before the expiration of their allowable time frames and those which exceed their specified time frames are reported to the Commission within the specified reporting time frame.

Until the cancellation of each of these LCOs, appropriate stationed fire watches will be maintained at the affected plant areas in accordance with technical specifications.
