Dear Mr. O'Reilly:

This confirms the conversation between Mr. R. M. Poulk, Jr., Regulatory Compliance Specialist, and Mr. C. A. Julian of your office at 1500 on August 28, 1981, concerning an immediately reportable event per Technical Specification 6.9.1.8b.

At 1400 on August 27, 1981, 160 personnel wrote a trouble ticket on failed llPCI temperature switch E51 dTS-N604D and delivered it to Operation? '

' personnel. Failure of this instrument requires that it b. repaired within two hcurs or IIPCT declared inoperable and isolated. Operations personnel did not issue this trouble ticket to be worked until coproxi-mately 1900 on August 27, thereby exceeding the two-hour time requirement for instrument . ropair or declaring HPCI inoperable. - HPCI was immediately isolated and repairs to the instrument were made. The HPCI System has now been returned to service and an investigation of this event is underway.
