21.21 UNSPECIFIED PARAGRAPH PART 21 REPORT - POTENTIAL MINIMUM WALL VIOLATION ON SEISMIC PLATE VALVE BODIES "While reviewing Valve Body drawing 16B2201, it was discovered that an area in the packing bore has the possibility to not meet the minimum wall requirements of ASME B16.34 for a Class 150 valve at the minimum material condition due to a potential stack up of tolerances.

"Upon discovery of this possible failure to comply, an investigation was performed on all NPS 3-12 Type Al1 Class 150 Seismic Plate Valve Bodies and it was determined that this issue was isolated to the NPS 8 Type Al 1 Class 150 Seismic Plate Valve Body per drawing 16B2201.

"It is the opinion of Fisher that there is no inherent safety risk associated with this situation. Fisher will be working with the individual plants [Clinton Power Station and Watts Bar] in question to measure the actual dimensions of the bodies to determine if a problem exits and will take appropriate corrective action."

Purpose:

The purpose of this Fisher Information Notice ("FIN") is to alert customers that as of February 15, 2012 Fisher Controls International LLC ("Fisher") became aware of a possible failure to comply.

Pursuant to 10 CFR 21.21(b), Fisher is providing the required written notification of a failure to comply.

Receipt of this notice does not necessarily mean that the recipient has been shipped any of the subject equipment. Fisher records indicate that only the listed serial numbers are the affected units. It is expected that the recipients of this notice will review the information for applicability to their facilities, and if required, take the appropriate action as described in the section at the end of this notice.

Applicability:

This notice applies only to the equipment identified in the table above. The table lists Fisher Order and Item Numbers that were delivered to Exelon and WVA. No other items were affected.

Discussion:

While reviewing Valve Body drawing 1682201 (NPS 8 Type All I Class 150 Seismic Plate Valve Body), it was discovered that an area in the packing bore has the possibility to not meet the minimum wall requirements of ASME B16.34 for a Class 150 valve at the minimum material condition due to a potential stack up of tolerances.

Upon discovery of this possible failure to comply, an investigation was performed on all NPS 3-12 Type All Class 150 Seismic Plate Valve Bodies and it was determined that this issue was isolated to the NPS 8 Type All1 Class 150 Seismic Plate Valve Body per drawing 16B2201.

It is the opinion of Fisher that there is no inherent safety risk associated with this situation, Fisher will be working with the individual plants in question to measure the actual dimensions of the bodies to determine if a problem exists and will take appropriate corrective action.

Fisher will revise the drawing to ensure that this issue is corrected.
