Box296 10 Austin Avenue Greenville, RI 02828 Tel: 401-949-4733 Fax: 401-949-0220 January 4, 2019 Degraded Snubber SF1154 Hydraulic Fluid Batch No. 11 KLVS 146 The following table reflects the status of US nuclear plants and other facilities to which SF1154 fluid from Batch 11KLVS146 was shipped by Lake Engineering Company (LEC) in accordance with purchase orders which specified that the requirements of 10 CFR Part 21 are applicable.

Fluid was originally provided to Wyle Laboratories (currently NTS) and was subsequently installed in snubbers at Indian Point 2.

Lake Engineering Company shall continue in its evaluation to determine whether the subject anomalies represent defects which could create a substantial safety hazard. Determination in this regard shall continue to involve interaction between Lake Engineering Company and the facilities listed above.

Our current goal is to complete our evaluation by February 28, 2019 at which time a determination will have been made as to whether a substantial safety hazard exists. If additional time is required for the above determination, an interim report shall be submitted that shall include an updated completion date.
