> e d LER 251-82-007 ADDITIONAL EVENT DESCRIPTION AND PROBABLE CONSEQUENCES Unit 4's Boron Injection Tank-(BIT) was being recirculated with C Boric Acid Storage Tank (BAST) by using the mini recirculation pumps. The BIT was sampled at 5:00am and the results showed a concentration of 19,000 ppm. Another sample was taken to check the results of the first analysis. The original results were confirmed. The system lineup was checked and flow was adequate. Then C BAST was recirculated alone and a sample was analyzed. A low boron concentration was also found in the BAST.
ADDITIONAL CAUSE DESCRIPTION AND CORRECTIVE ACTIONS The system lineup was inspected and MOV-4-867B was found open one hand turn.
The nuclear operator closed it manually. No other abnormalities were found in the system. While the BIT-BAST recirculation was taking place, a high head safety injection pump was being used to fill the accumulators on Unit 3 with water from the refueling water storage tank. It is suspected that some of the flow went through MOV-4-867B, diluting the BIT. However, no substantial level increase in the BAST was noted that would tend to support this explanation.
After MOV-4-867B was closed, the BIT was recirculated with one of the other two BASTS which contained adequate boric acid. At 7:15am, a sample from the BIT was analyzed and found to have sufficient boron concentration. Boric acid was added to C BAST and an acceptable concentration was confirmed at 12:19am on May 23, 1982.
Apparently, the Limitorque Operator on MOV-4-867B had failed to close the valve completely. A test will be devised to verify that the Limitorque operator properly closes the valve and assure that any seat leakage is min-imum.
It should be noted that with the upcoming installation of model 44F steam generators currently scheduled for November, 1982, the concentration require-ments in the BIT will be dropped.
