: 1. Summary Quench spray, recirculation spray and auxiliary feedwater pumps are being manufactured by Bingham-Willamette Company for Beaver Valley Power Station - Unit No. 2. To date, Bingham-Willamette Company has been unable to demonstrate that it has performed welding and non-destructive testing to approved procedures. Bingham-Willamette has in several cases provided documentation which fails to meet specifi-cation requirements. Pumps tested to date fail to meet the specification design and operation requirements. From the data provided to date, we believe that a reportable deficiency may exist as a result of the breakdown of Bingham-Willamette Quality Assurance Program.

BEAVER VALLEY POWER STATION - UNIT NO. 2 INTERIM REPORT ON THE "STOP WORK DIRECTIVE" FOR THE BINGHAM-WILLAMETTE COMPANY SIGNIFICANT DEFICIENCY 79-01

1. Summary Quench spray, recirculation spray and auxiliary feedwater pumps are being manufactured by Bingham-Willamette Company for Beaver Valley Power Station - Unit No. 2. To date, Bingham-Willamette Company has been unable to demonstrate that it has performed welding and non-destructive testing to approved procedures. Bingham-Willamette has in several cases provided documentation which fails to meet specifi-cation requirements. Pumps tested to date fail to meet the specification design and operation requirements. From the data provided to date, we believe that a reportable deficiency may exist as a result of the breakdown of Bingham-Willamette Quality Assurance Program.
2. Immediate Action Taken A "stop work" order was issued to Bingham-Willamette Company on March 29, 1979 for the Beaver Valley Power Station -

Unit No. 2 orders only. This "stop work" was issued of Bingham-Willamette's inability to demonstrate that work was done to approved procedures, provide proper documentation of NDE results, and effect corrective action specified in the disposition of Non-Conformance and Disposition (N&D)

Reports.

3. Description of Deficiency Bingham-Willamette failed to demonstrate compliance with the specification requirements for post-weld repair heat treating, welder qualification, welding procedure qualifi-cation, nondestructive testing procedure qualifications, material certifications, and pump operational test requirements.

Bingham-Willamette continued to manufacture the subject pumps for a period of 12-15 months before taking exception to the corrective action specified in Nonconformance and Disposition Reports. This lack of conformance to specified corrective action may have resulted in components which are either technically unacceptable or whose quality cannot be conclusively demonstrated.

4. Analysis of Safety Implication Pumps supplied by Bingham-Willamette include those for containment quench and recirculation spray and auxiliary feedwater systems.

Failure of this equipment to perform to specified require-ments may impact capabilities for containment and primary system heat removal respectively.

5. Corrective Action to Remedy Deficiency Corrective action has been submitted by Bingham-Willamette and is currently being reviewed by Stone and Webster Engineering.

The Duquesne Light Company and Stone and Webster Engineering, Corporation Quality Assurance Departments will perform an in-depth audit of the Bingham-Willamette Quality Assurance Program with respect to Beaver Valley Unit 2 purchase orders in early May, 1979. Demonstration by Bingham-Willamette of acceptable technical corrective action and substantiation of quality requirements may allow a final determination that no reportable deficiency, under the terms of 10CFR50.55(e) exists.
