The information is as initially received without verification or evaluation, and is basically all that is known by the Region II staff on this date.

UNSCHEDULED SHUTDOWN IN EXCESS OF 72 HOURS At 8:19 p.m. on November 15, 1993, Unit 2 tripped from 95% reactor power.

The steam-flow / feed-flow mismatch coincident with low steam generator (SG) level reactor trip occurred when all three main feedwater regulating valves (FWRV) closed. The FWRV closure resulted from a loss of control power to the "A" train solenoids when their supply breaker tripped open.

Plant response to the transient was as expected except the "A" auxiliary feedwater (AfW) pump had to be secured. The AFW pump will be repacked and the cause for the breaker trip will be investigated and repaired.

Since the May 1993 refueling outage startup, the unit has operated at power reductions due to level oscillations in the C SG. The licensee has elected to take the unit to cold-shutdown to investigate the cause of the SG level oscillations. The licensee anticipates that the outage will last approximately 17 days.

The Senior Resident Inspector was onsite performing a backshift inspection at the time of the trip. The SRI reviewed the licensee evaluations and activities and will continue to review the corrective actions.

The State of Virginia was informed.
