. This preliminary notification constitutes EARLY notice of events of POSSIBLE safety or public interest significance. The information is as initially received without verifi-cation or evaluation, and is basically all that is known by the Region II staff on this date.

SUBJECT:

SECONDARY SYSTEM PIPE FAILURE - UPDATE This PN updates PN0's II-86-91/91A/91B and 91C.

The licensee reports that two additional deaths have occurred as a result of injuries sustained by workers on December 9 when an 18-inch feedwater system suction line broke, scalding eight workers with hot water and steam. This brings the number of deaths to four. -

One worker remains hospitalized in critical condition, and one is reported to have [

evidenced some improvement. Two others were treated and released following the incident.

Both headquarters and Region II personnel are continuing to follow up on licensee actions associated with this event.

The NRC will issue an Information Notice this week to all licensees potentially affected by this phenomenon.

Both the licensee and the NRC are continuing to respond to media inquiries.

The Commonwealth of Virginia is being kept informed.

This information is current as of 10:00 a.m. (EST) on December 16.
