Joseph M. Farley Nuclear Plant, Unit 2 Special Report 201S-001-00, Nonfunctional Radiation Monitor A-SOC Enclosure Special Report 2016-001-00 This Special Report is being submitted due to the Unit 2 radiation monitor R-S08 and A-SOC (Main Steam Relief and Atmospheric Steam Dump Discharge) not being restored to functional status within the time required by Technical Requirements Manual (TAM) TR 13.3.4. The TAM requires that the monitor be restored to functional status within 7 days or submit a Special Report within the following 14 days.

At 0439 on May S, 201S, with Unit 2 In Mode 3, radiation monitor R-S08 and A-SOC were declared nonfunctional due to a failed high indication on the control panel. A mandatory Technical Requirements Manual (TAM) Limiting Condition for Operation (LCO) was written requiring restoration to functional status by May 13, 201S at 0439 hours0.00508 days <br />0.122 hours <br />7.258598e-4 weeks <br />1.670395e-4 months <br /> and work orders (SNC78803S & SNC788035) were generated to repair R-S08 and A-SOC.

Troubleshooting of the failed high indication isolated the problem to the detectors.

Due to the detectors' location on the main steam valve room roof being in close proximity to steam generator safety relief valve exhaust piping, repair of the monitor is a personnel safety concern should any relief valve lift during the repair effort. Therefore, repair of the monitor should be performed only under cold shutdown conditions to ensure personnel safety. Contingency work orders are in place in the event of an unscheduled outage or the next refueling outage. R-S08 and A-SOC are scheduled to be retired in place and replaced with new radiation monitors, In a location that will allow for detector repair at any mode of operation, during the next refueling outage. Compensatory measures have been established and TAM action requirements are being met for radiation monitors R-S08 and A-SOC nonfunctionality.
