Joseph M. Farley Nuclear Plant, Unit 2 Post Accident Monitoring Report Enclosure Post Accident Monitoring Report This Report is being submitted due to the Unit 2 Containment Sump Level Indicator L T3594A not being restored to operable status within the time required by Technical Specifications (TS) 3.3.3. The Required Action Statement requires that the indicator be restored to operable status within 30 days. If not restored within the 30 day period, a Post Accident Monitoring Report is required to be submitted within the following 14 days per TS 5.6.8.

Background Information:

At 0439 on May 6, 2016, with Unit 2 in Mode 3, Containment Sump Level Indicator LT3594A was declared inoperable due to a failed low indication. A mandatory Technical Specifications Limiting Condition for Operation (LCO) was written requiring restoration to operable status by June 5, 2016 at 0439.

Cause of the lnoperability:

A work order was generated to troubleshoot L T3594A. Troubleshooting of the failed low indication iSolated the problem to the power supply cable. Causal analysis identified that component aging was the cause of the failure of the power supply cable.

Alternative Monitoring Method:

There are two separate indicators available to provide Containment Sump level indication. Containment Sump monitoring is still available using the opposite train indicator L T35948. TS Required Action Statements are being met for Containment Sump Level Indicator L T3594A inoperability.

Plans and Schedule for Restoration:

The troubleshooting work order was revised to perform replacement of the failed power cable and will be completed per the Work Management process.
