1. External Inspection completed 8/24112. Internals Inspection and anchorage vetfilcatNon performed 1M9/13.2. As noted In Question 4. several of the anchor bolts are missing grout In a cone-shaped configuration.

This was noted In the SEWS package (&24194) and a subsequent delaied evaluation showed this condition does not present an Immediate operablity concern. Several enhancements have been made to the anchorage incudlag:

shlmdrrng a gap between the panel and grout pad; additIonal 3S expansion anchors on the back (North) side of the cabinet; and a 114' plate welded to the frame which Is anchored by (2) 3/8" expansion anchors near the font (South) side of the panel. Therefore, this condition Is judged by the SWEs to not be an Immedat concern. However, repairs should be made to the existing grout near the cast-in-place anchors.(Ref. The HSDP contains a large amount of active and spare cables. All cables were observed to be bundled together in sections wih several large plastic tie wraps and contain adequate flex with their current routing. The tie wraps siue are large enough to not prevent a risk of cutting the cables during a seismic event No adverse seismic conditions have been Identifitd.

All front and rear panels were removed and Inspected; however, a second, internal panel was not removed In the rear of the swltchgear.

A review of the associated SEWS packages for 4.16 kV Switchgear shows that QIR15AS04 (WJ) Is bounded by SEWS package for Q 1R15A505 (1K), which is the governing package for all 4.16 kV Swltchgear.

As stated in the SEWS package for Q1R15A503 (1H), additional Inspections were performed to provide a level of confidence that all 4.16 kV Switchgear were installed properly.

An additional conservative anchorage calculation (S0-93-2-8685-001) was performed for six other similar swltchgear using GIP criteria to prove all units had acceplable anchorage.

In total, eight switchgear had visual verification of anchrage which provides a high level of confidence that all swltchgear were Installed properly.3. SWC performed and signed originally on 10/11/13.

Revised and re-signed to provide additional Information on anchorage verification.
