procedure and responsibilities for preparation, review and approval of Pump and Valve Operability Packages (PVOP's).

This procedure also establishes the minimum contents of each PVOP.

~2.0 ~ ~ Scope ~ ~~~~

This Project Instruction applies to all safety-related,

" active" Seismic Category I pumps and valves.

3.0 Definitions 3.1 Pump & Valve Operability Packace (PVOP) - A package, the contents of which are detailed in Section 4, that compiles the documentation and/or references necessary to demonstrate the ability of a safety-related, active, Seismic Category I pump or valve to perform its active safety function. A package may document the operability of a single component or a number of similar components.

3.2 Safety-Related,-Active Pumos and Valves - A pump or valve which must perform a mechanical motion in order to shut down the plant or mitigate the consequences of a postulated event. Safety and relief valves are specifically included.

3.7 Pump & Valve Operability Review (PVOR) Master List - A n complete list of safety related, active Seismic Category I

' Each PVOP shall consist of a Specification Section and one or more System Sections. Each Specification Section shall address the scope and technical requirements of a single procurement specification. Each System Section shall address the specific qualification requirements of one or

' ~ ~ ~ ~ ~ more components.___The. determination _of how a number of components may be combined into one System Section shall be based on the compatibility or similarity of the component's design.and functional requirements and resultant qualification requirements.

Each PVOP shall conform to the following format:

4.1 Specification Section will include the following:

4.1.1 PVOP approval and issue summary page (Attachment C).

4.1.2 Table of Contents (Note 2). l 4.1.3 Subsort of PVOR " Master List" identifying all pumps or valves in the package (Note 2). l 4.1.4 Technical Requirement Section of the Procurement Specification (Note 1).

l 4.1.5 IPC Letters S-5568 and S-5569, dated 7/5/85 regarding plant  ;

Pre-Operational Test Procedures and plant preventive maintenance, respectively (Note 1). l 4.1.6 Any other procedures or information relevant to operability for every component in the package (Note 1).  !

5.5.4 Providing the PVOP Preparer with the reviewed Vendor Test Reports and associated checklists.

6.0. Procedure 6.1 The System Engineers shall prepare a list of all safety-related active, Seismic Category I pumps and valves for the systems assigned them by the Project Manager. The System Engineer shall sign and date the list as prepared. The System Engineers shall forward'the prepared list to the System Reviewer identified by the Project Manager.

6.2 The System Reviewer shall review the list to assure that it is technically correct, complete, and consistent with the definition of active stated in this project instruction.

The System Reviewer shall resolve any comments with the System Engineer and, when satisfied, sign and date the list as reviewer.

6.3 The prepared and reviewed list shall be approved by the Project Manager (or his designee). A copy shall be transmitted to the System Engineers, System Reviewers, the CQD Senior Project Engineer, and the OA Records Section in accordance with GO-17.02 (Ref. 7.1).

6.4 A revision to this list shal,1 be initiated by the System Engineer to add or delete, as necessary, components based on changes to system design and function. Any required revisions to the Final Safety Analysis Report shall be identified to the Project Manager.

This designation may subsequently be revised based on the more detailed review of the PVOP Preparers l.

performed in Sections 6.8 thru 6.16.

6.8 The CQD Project Engineer, the C&ID Engineer, and the Vendor Test Data Review Coordinator shall prepare their input (as defined in Section 5 of this Project Instruction) and forward the same to the PVOP Preparer.

6.9 When completing or providing input to the completion of the PVOR Checklist, the engineer shall:

1) consider the appropriate licensing, regulatory, and '

industry code or standard requirements applicable to the pump or valve's operability. A matrix of these requirements is included as Attachment A.

2) provide a specific reference to the document (i.e.,

analysis report or test report) that demonstratea satisfactory compliance to the operability requirement.

6.10 The PVOP Preparer shall assemble input from the interfacing divisions and prepare the PVOP.

6.11 The PVOP Preparer shall identify on the transmittal cover page ( Attachment D) any deficiencies, or inconsistencies in the N information contained in the PVOP. This shall include all

. items which must be closed out before the final PVOP is

$ approved. One copy of the transmittal cover page chall m remain with the PVOP and one copy should be sent to the Project Manager or his designee.

. punchlist of all PVOP. deficiencies and inconsistencies as y identified on the transmittal cover page. This punch list g shall be used to' insure that all open items are closed out before the final PVOP is approved.

c ..c . c o REV. 2 6.13 The PVOP Preparer shall sign and date the PVOP as preparer and forward it to the PVOP Reviewer.

6.14 The PVOP Reviewer shall review the package for technical adequacy and completeness. Any comments shall be reviewed with the PVOP Preparer and, when comments have been resolved to his satisfaction, the PVOP Reviewer shall sign and date

6.15 After the PVOP is reviewed, it shall be transmitted to IPC by the Project Manager (or his designee). IPC will then perform a walkdown of the component in the PVOP and transmit the walkdown results to S&L.

6.16 After receipt of the walkdown results from IPC, the PVOP Preparer shall compare the walkdown information with the PVOP. Any discrepancies shall be resolved with IPC, documented and filed. If revisions to the PVOP are l necessary, it shall be reprepared and reviewed in accordance with Sections 6.8 through 6.14. An additional walkdown by IPC is not required for such revisions.

6.17 Completed PVOP's shall be submitted to the Project Manager (or his designee) for approval.

6.18 Approved packages shall be submitted to IPC under S&L cover letter by the Project Manager (or his designee). Copies of the packages shall be maintained in S&L's Mechanical .

Department File.

Components said to be qualified and installed have been turned over to Startup for testing. Audit results indicate the qualification package is not for the installed component. Review installed equipment verses the qualification in packages to assure this is not a generic item. Upon completion of this review, provide findings and results.

Illinois Power's response is as follows:

Through walkdown of packages Illinois Power Company (IPC) has determined that this is an isolated case and not a generic problem. Walkdowns will continue on schedule for all the PVORT packages. The components in each qualification package will be walked down by IPC prior to Sargent and Lundy approval of the package.

Anchor Darling's response to this concern is contained in the s attached TLX. The acceptance criteria for the disc test, per the TLX, is the same as the acceptance criteria for.the. seat test.

These tests are one in the same.

The maximum allowed seat leakage has been verified to be 100 cc/hr and not 0 cc/hr as originally recorded on the A/D Data Report. For this application 100 cc/hr is acceptable according to MSS-SP-61 " Manufacturers Standard Specification for Pressure Testing Steel Valves". MSS-SP-61 is referenced by Sargent &

. Lundy (S&L) Standard 271, which is referenced by Piping Design Table 908CP. Piping Design Table 908CP is contained in Valve Specification K-2866A, ASME Section III Valves 2 inch and larger.

. Hence,-the 24 cc/hr leakage during the disc test is acceptable.

The only condition that would require rapid. closure is a circumferential pipe break on the HPCS pump side of the' valve. However, because this portion of

.the system operates at high energy levels less than 2% of the. time (as referenced in the Standard Review Plan Section.3.<6.2, Branch Technical Position MEB 3-1, Para.

MWR B-16519 was initiated by the IP-Startup System Engineer on June 24, 1985. During performance of the Generic Test Procedure

-(GTP-11, Air Operated Valves Non-Modulating) the engineer observed

.the disc shaft was slightly bent. The Startup engineer then pro-ceeded to cycle the valve.as pequired by GTP-ll, and noticed the valve did not operate smoothly. When the valve was opened with the air operator, there was an excessive amount of time from initiation to full open. In accordance with plant procedures (CPS 1029 01, Preparation and Routing of Maintenance Work Requests) MWR B-16519 was initiated identifying the above problem.

Maintenance on valve lE22-F005 was then initiated at this time as a' result of the MWR. IPC Maintenance reworked the valve in the CPS machine shop and the valve internals were reinstalled. Generic testing of the valve and completion of GTP-11 will be done to confirm the valve functions per design.

'that on May.1, 1981, Limerick issued Significant Deficiency Report No. 38 on Defects-in-Limitorque Valve Motor Operators.which, in part, addressed cracks in limit switch rotors manufactured of Melamine.

Cracking of limit. switch _ rotors on Limitorque valve operators has been addressed at CPS by Condition Report 1-84-02-047. The following actions have been taken to

ensure that the cracking of Melamine limit switch rotors will not impair the operation of safety related valves-with Limitorque motor operators and if cracking does' occur between inspections the safety related function of.the valve will~not be impaired: ' '

All safety related motor operated valves with Limitorque operators have been inspected and will be reinspected every eighteen months for cracked Melamine rotors.

MWRs-have been-written to replace all identified-cracked Melamine rotors.with Fibrite rotors which have'been authorized by the manufacturer and have not exhibited the potential for cracking.

3 ATTACHMENT 15A EQUIPMENT QUALIFICATION ANALYSIS VERSUS TEST 1The purpose of qualification is to demonstrate that safety-related components possess the capability to perform their safety-related function (s) -during and/or af ter the postulated design events for which these safety functions are required. As a result, the qualification must consider both the normal operating and design basis loadings to which the component may.be subjected.--Qualification ~is~ demonstrated by a program of test

In demonstrating the operability of safety-related pumps and valves for postulated seismic events, neither a program of testing alone or analysis alone can adequately address all of the parameters affecting a component's ability to operate. Consider the following examples:

A.

In seismic qualification testing of pumps (especially large pumps) it is not possible or practical to:

i) simulate full flow conditions in order to impose the maximum fluid dynamic forces upon the impeller,

11) apply the piping nozzle reactions to the pump casing, iii) mount vertical pumps on a seismic shake table.

B. Analytical techniques cannot reliably determine:

i) the susceptibility of a position limit switch to

ii) The amount of set. point drift that will occur with a pressure switch, iii) the ability of electronic circuits to withstand vibratory excitation, iv) the response of complex ; mechanical components which possess inherent non-linearities associated with gaps, slip joints, gear trains and the like.

In'accordance with these criteria, the operability of. safety-rel'ated active pumps and valves is demonstrated by the following

Qualification is demonstrated by analysis. The analytical model considers the actuator as a lumped mass. Stroking capability is demonstrated by computing' deflection of valve yoke which shows that lateral defleetions are small and thus preclude stem binding.

Analysis results are verified by testing representative samples of valve assemblies. Such testing consists of biaxial and triaxial, multifrequency shake _ table _ tests.and static pull tests

(~see Attachment 1).

B. 1 Pumps

1) Motor driven pumps Qualification is demon'strated by analysis.

Operability is demonstrated by deflection analysis which shows:

pump impeller will not interfere with pump casing,

- motor rotor will not interfere with the stator,

- alignment of pump-motor shaft at coupling satisfies vendor requirements.

The models and analytical techniques used have been '

substantiated by in-site. testing performed for other projects (see Attachment 2).

In addition, the life of the bearings is checked in accordance with the standards of Anti-Friction Bearing manufacturers Association (AFBMA).

ii) Turbine Dri.ve'n Pumps Qualification of the turbine is demonstrated by test

Sargent & Lundy (S&L) will review all qualification documents 1that are committed to in the FSAR, including a complete review of all previously GE responsibility components.
