Dear Mr. Stello:
This letter is provided to report a potential error condition in a computer software program which Control Data Corporation makes available to users of its CYBERNET computer system.
Control Data Corporation is unable to evaluate whether this error condition could create a " defect" as defined by 10CFR21, and, therefore, has notified its customers by letter dated July 7, 1982, in order that they may cause the error condition to be evaluated.
g The error condition exists in the computer program PIPESD ( tl:Le
" program") which can be used by Control Data customers to pef-form pipe stress analysis.
The program operates on Control Data's CYBERNET computer systems, under control of the SCOPE 3.4 and NOS operating systems at data centers located in:
Rockville, Maryland; Houston, Texas; Sunnyvale, California; Waltham, Massachusetts.
The program is the proprietary product of URS/ John A.' Blume &
Associates, Engineers, 130 Jessie Street, San Francisco, California 94105.
A list of customers who were notified is enclosed together with a copy of Control Data's notification letter which has been sent to each of the customers.
This list includes both customers who may have and may not have used PIPESD in nuclear work.
The nature of the error condition, and the corrective action which should be taken by users to avoid further errors, are described in the enclosed notice to customers.

Control Data Corporation would like to inform you that a potential error condition has been detected in the computer program PIPESD which is available on CYBERNET SCOPE 3.4 and NOS operating systems.
When modeling_an elsstic spring support,_the PIPESD user must specify several parameters including support grid point number, support type, support spring stiffness,.and choice of-' global" or " inclined" support direction.
In the. event that-1)

are specified on the same' support grid point number, PIPESD may incorrectly calculate the support reactions.
Results from time history load cases and response spectrum cases are unaffected; URS/ John A.
Blume and Associates has researched the extent of

this problem through earlier program versions.
It appears to exist as early as Version 4.0 of PIPESD (circa 1975) and probably as early as Version 3.0 (circa 1974).

-URS/Blume has identified a method of.a' voiding the problem until a correction can be released.
A description of this method is, enclosed as Attachment A.
In addition, Control Data may be able to assist you in determining whether your runs of PIPESD have been affected by this condition.

A revised version of PIPEbD is being prepared by URS/Blume to correct the problem which is herein reported.
This revised versio'n will be made available to Control Data's CYBERNET users as soon as possible after it is received from URS/Blume.
Until that time, the workaround described in Attachment A should be used.

BOARD NOTIFICATION N0. 82 The results of a Region IV investigation at Teledyne have been sent to the appropriate Board and parties for Diablo 1 & 2, Fermi 2 and Surmier.
The investigation was stimulated by an allegation relating to the Fitzpatrick and Yankee Electric facilities. The results reflect an audit of the Teledyne internal QA program.
Since the investigation may be viewed as a cross-cut of the Teledyne QA program, its results reflect the general level of confonnance to QA requirements at Teledyne.
It is therefore applicable to all plants for which Teledyne is doing work.
These plants include those listed above in addition.to Limerick.
For those plants which have a hearing, but for which the hearing has not yet started, please ensure that the substance of this board notification is included in your SER or SSER.
It is the opinion of the staff that this report does not constitute a major safety concern.

SUBJECT:
DIABLO CANYON DESIGN VERIFICATION PROGRAM - PHASE II On November 19, 1981, the Commission issued an Order (CLI 81-30) that suspended the license for fuel loading and low power test operation for Diablo Canyon Unit 1.
The Commission's Order sets forth the requirements for a design verification program that must be satisfactorily completed prior to the restoration of that license.
The activities necessary. to comply with the requirements of the Order have become known as Phase I of the Diablo Canyon Design Verification Program.
On the same date, the staff issued a letter to Pacific Gas & Electric (PG&E) that sets forth requirements that must be completed prior to the staff recom-mending operation at a power level above 5%.
The activities that comply with the requirements of the staff letter have become known as Phase II of the Diablo Canyon Design Verification Program.
The program plan for Phase II of 'the Design Verification Program was sub-mitted by Teledyne on June 19, 1982.
The staff has requested, by letters dated July 8, 1982, the Governor of the State of California and the Joint Intervenors to provide their comments on the proposed program plan. The Phase II program plan is under review by the staff and is expected to be completed in early August.
In addition, the staff is reviewing a number of Interim Technical Reports resulting from Phase I activities.
A meeting is being scheduled in late July to discuss with Teledyne and PG&E the results of an independent design analysis of the Diablo Canyon Unit I containment annulus structure performed by the staff consultant, Brookhaven National Laboratory.
Consideration is being given to a meeting with PG&E and Teledyne in early August to discuss the status of Phase I and II activities.
