This report documents in assembled form all the pertinent observationsmade by our inspectors during the CO reviews at the site and in San Jose of the recent control rod drives crud problem.
This matter has received close review by the' Regulatory l
and has been the subject of correspondence and a meeting between the licensee and the Regulatory.

You will notes that one control rod, No. 30-03, has been declared j
inoperable due to a suspected bent coupling' shaft.
Correction will require. removal of the rod from the reactor, scheduled for -
the.first refueling outage.

Followup items identified in the report will be addressed in subsequent inspection reports as is appropriate.
