l. tilTRODUCTl 0:1 In accordance with Technical Specification 6.6.B.1 for Surry Power l

Station, Operating License DPR-32, this report describes an abnormal occurrence which occurred on April 15, 1974. The Directorate of Regulatory Operations, Region 11, was notified on April 16, 1974.

The occurrence reported herein is classificd as an abnormal occurrence pursuant to Technical Specification 1.0. l .7 which states that "An abnormal occurrence is defined as: Any observed inadequacy in the implementation of administrative or procedural controls during the operation of the facility which would significantly affect the safety of operations."

. The occurrence described herein involved a failure to include a step in an operating procedure to ensure reset of the emergency dicsci auxiliary trip reset pushbutton. .

Il. SUMitARY 0F 0,CCURREllCE On April 15,1974, OP-06, Emergency Diesel ' Generator, was -performed i

on the Unit tio.1 emergency diesel generator to verify operability, j.

Step 2.13 of the initial conditions required that the auxiliary trip reset pushbutton be depressed, verifying that the amber light on the control panel was out. The Unit tio.1 operator. noticed that the amber light was out and assumed that-the reset pushbutton had been denressed.

-Upon trying to synchronize the diesel to the system,- the generator breaker failed to close.

O lli. AllALYSIS OF Tile OCCURREtiCE The generator breaker was kept open due to an energized breaker trip coll. Investigation showed that relay EGl-TX* was energized and scaled in, thus maintaining a trip signal on, the circuit breaker. The

- amber light, normally energized when -the EGl-TX relay is energized, was found to be burned out. By depressing the auxiliary trip reset pushbutton, relay EG1-TX was de-energized, and the generator was synchronized to the system.

During the performance of PT-22 3C, which incorporates OP-06, the EG1-TX relay is energized and scaled in. it was discovered that the procedure failed to ensure the return of the circuit to normal by not including a step to reset the auxiliary trip pushbutton following completion of the procedure.

The investiaation to determine the reason for not incorporating the reset function into the procedure revealed that the reset function was added after the original installation. At the tims of this modification, comprehensive design change procedures were not in effect at the station.

The design change procedure was implemented in July 1973, prior to the modification. If the procedure had been in effect at the time the nodification under discussion was made, it would have reduced the probability of the event occurring.

o change has been initiated to provide an additional annunciator alarm for emergency dicsci generator protection to monitor the EG1-TX relay Further action was taken to change the operating procedure to require the operator to reset the pushbutton after performing OP-06 and so noting by his initials.

As stated above, a detailed procedure for processing station design changes or modifications is no.v in effect. This procedure provides for reviewing the operational aspects of facility design changes. The re fore ,

the management controls presently in effect should. reduce the probability of a recurrence of a similar incident. in addition, an annual review is conducted for all operating procedures.

V. ANALYSIS AND EVALUATION OF SAFETY IMPLICATIONS OF THE OCCURRENCE _

V I '. CONCLUSIONS The licensee concludes that:

1. The operating procedure has been corrected so that the auxiliary trip reset pushbutton.must be reset following the completion of OP-06.
